Information We Receive
EPI-Q receives anonymized information collected by its clients and research providers for analysis. This may include receipt of health information collected by providers of healthcare services and other health care data vendors. Providers of data to EPI-Q must certify that information was collected under all relevant local, regional and national regulations, and with appropriate informed consent, where applicable. Data received by EPI-Q from its clients and research providers is anonymized, in that it is stripped of elements of Protected Health Information (PHI) under the United States (US) Health Care Insurance Portability and Accountability Act (HIPAA) and Personal Data under European Union (EU) General Data Protection Regulation (GDPR).
EPI-Q collects primary data through health care patients’ voluntary participation in surveys. Data provided via surveys may collect elements of PHI or Personal Data upon EPI-Q’s receipt of patients’ informed consent. EPI-Q’s survey data collection servers are located in the US, and survey participants outside the US will be made aware in the informed consent process that their Personal Data may reside in the US. EPI-Q maintains partnerships with survey providers with servers located in the EU. Upon our client’s request, EU-based servers may be used to collect and store survey data. Data transferred to the US from EU servers will be fully anonymized and will be made with appropriate agreements in place.
Via our website, we collect information from people who use the online form called “Contact Us”. When contacting us, individuals are asked to enter: name, e-mail address, mailing address, or phone number. Visitors may access our site anonymously. Our website server is located in the US and users will be made aware of this via accessing this privacy policy.
How Do We Use this Data
Anonymized information from our clients and research providers, as well as data collected via surveys are aggregated and analyzed in the context of our health economic and outcomes research services. Survey data is anonymized prior to analysis. Aggregated analysis results are presented in reports to our clients and research partners. Aggregate, anonymized analysis results may be published in medical journals or presented at medical conferences.
Data collected from our website may be used to send periodic emails. Email addresses provided at registration may be used to send occasional company news, updates, related product or service information.
Disclosing Information
EPI-Q does not disclose or sell PHI/Personal Data with any third parties. EPI-Q may disclose Personal Data in response to a lawful request by public authorities.
Accessing Your Information
Individuals that have given EPI-Q consent to receive and have provided PHI or Personal Data to us may review that information maintained by contacting info@epi-q.com. Individuals that identify errors in their personal information may submit corrected information to EPI-Q.

We use cookies on our website only to determine the functionality of internet navigation. No Personal Data is stored in any cookie.

Opting Out
Individuals that have provided consent for EPI-Q to collect self-reported survey research data may withdraw that consent at any time without penalty. Any data collected prior to withdrawal of consent including the name of the individual providing informed consent, may remain in the survey dataset. This is disclosed in the initial consent process.
Website users can unsubscribe from receiving future emails and we include detailed unsubscribe instructions at the bottom of each email. Upon unsubscribing, Personal Data will be removed from our database.
Data Security
EPI-Q takes reasonable steps to maintain the security of the personally identifiable information that we collect, including limiting the number of people who have physical access to our servers, requiring password authorization to access data, maintaining electronic security systems that guard against unauthorized access. However, no data transmission over the Internet can be guaranteed to be completely secure. Accordingly, we cannot ensure or warrant the security of any Personal Data that you transmit to us, so you do so at your own risk.
Compliance
If you have a complaint regarding a breach of your Personal Data, EPI-Q must respond to you within 45 days. Complaints should be forwarded to info@epi-q.com.
